TRANSITION SERVICES AND TRANSITION PROGRAM

The purpose of the transition program is simple: help individuals with developmental disabilities to reach maximum independence as they grow older and make sure the families of those individuals are well informed about all services to which they are entitled.

We understand the complexity of planning and preparation. Services are available even if the learner is over the age of 21.

The Transition Program consists of a team that will take a hands-on approach to assess and create a comprehensive plan for the learner. The members of the Transition Team hold great experience working with both children and adult populations within a variety of program settings including independent living arrangements community-based habilitation programs, and vocational skill training programs.

The Transition Team will schedule visits, observe and assess the learner and hold Transition Guidance Meetings using person-centered planning. The team develops realistic adaptive goals which are firmly rooted in the principles of Applied Behavior Analysis (ABA). The activities will specifically focus on essential skills to foster independence at home and in the community. These goals are also incorporated into a written Transition Plan. All team members will be available to attend external service meetings to advocate for the learner and family.

Transition Coordinator under the Transition Services

The Transition Coordinator acts as a liaison for linkage and referral to services and a resource for the team working with the individual. The Transition Coordinator will provide important information to parents about service eligibility, events, and regulations. School visits and home visits are conducted to ensure continuity of post-secondary goals and activities. This set of coordinated activities ensures that the student and family have access to the appropriate resources as the student gets older.

Transition Services to generate a Transition Plan

Transition Guidance Meetings are also held by the Transition Coordinator, providers and the family. The ultimate goal is for the coordinator to generate a Transition Plan highlighting realistic, post-secondary goals.

The Community Life Skills Trainer's goal is to complete a comprehensive Transition Evaluation for each learner. In order to administer this testing, the Community Life Skills Trainer must coordinate with the providers and family. Once the assessment is complete, the Community Life Skills Trainer will work with the providers and the student to develop and implement functional goals. The trainer will also be responsible for contributing to the student's Individualized Transition Plan, and Measurable Post-Secondary Goals. Our Life Skills Trainers hold backgrounds in various background such as Social Work and Special Education Services and Behavior Analysis.
